Hospital Services Division
This division functions by helping healthcare providers to emplace, on-site or near-site, a comprehensive network of preventive, primary, home health, mobile diagnostic services and acute care services - and by actively, and appropriately, encouraging seniors to take advantage of the informational, preventive, diagnostic and treatment programs we offer. These programs include health promotion, screening/preventive care, primary geriatric healthcare, specialty care, case management and patient transportation. We also provide strategic planning to help hospitals increase revenues for geriatric outpatient services.
In recent years, we have provided services in Florida, Kentucky, New Jersey, New York and Pennsylvania. Our clients have included: Beth Israel Medical Center; Flushing Hospital Medical Center; Jamaica Hospital Medical Center; Hackensack University Medical Center; Maimonides Medical Center; Montefiore Medical Center; Mount Sinai Medical Center; the Orlando Regional Health System; the Saint Barnabas Health Care System; University of Louisville Hospital.
Public Agencies and Medicaid Division
The ElderCare Companies have developed a comprehensive array of effective socio-medical and environmental interventions designed to reduce the frequency, severity and costs of falls among the elderly.
ElderCare has successfully operated multi-site, multi-year, multi-million dollar fall prevention demonstration projects for Florida and Pennsylvania Medicaid since June of 2000. We have operated similar-but-smaller projects for County and municipal governments in New York and New Jersey since 1996. In Pennsylvania, Florida and New York we have been able to reduce: injurious falls by 54%; fall-related hospitalizations by 55% due to fracture and 64% for all fall consequences; nursing home stays by 65%, cost-per-case for Medicaid by 57% for nursing homes, 21% for hospitalizations, 32% for rehab and 75% to 80% overall.
In our Medicaid and related projects, we work closely with Medicaid HMOs and with nursing home diversion/dispersion/avoidance programs.
The final data of our Florida Medicaid project and Pennsylvania Medicaid project were considered when we were nominated for one national prize in 2005 and won a second national prize in 2006.
Post-Fall Counseling Division
ElderCare’s unique intervention to prevent second and repeat falls via a set of actions reimbursable by third party payors under OT, PT and behavioral health codes is being eagerly pursued by major U.S. home health and rehab companies - - because we can document an ability to reduce repeat falls by 75% in soon-to-be-published studies. This division is ElderCare’s chief point of contact with the nursing home or long term care industry and is a point of contact with HMOs.
We are offering to do on-site post-fall counseling in nursing homes as a provider in selected States and to manage/arrange/facilitate such services in all States.
Insurance, HMO, Payor Division
ElderCare has provided various services to Medicare HMOs in New York and New Jersey for more than five years. We are now being asked to enhance our services and to provide them to Medicaid HMOs, long term care insurers, commercial insurers (for covered retirees and for the rapidly growing number of older workers) and for property, casualty and liability carriers.
Federally Qualified Health Centers (FQHCs) Division
The ElderCare Companies is active in assisting FQHCs to increase their geriatric service volume and thereby helping to transform these traditionally under funded agencies into viable, profitable, community-based practice management corporations.
Mobile Geriatrics Division
The ElderCare Companies, Inc. has recently developed an innovative program to work with hospitals to provide outreach and/or management services for mobile medivans designed for geriatric diagnostic and treatment services. Our vans, and programs, are state-of-the-art highly customized to capture both primary and specialty diagnostic and treatment revenues. We believe our programs will rapidly sweep across the nation.
